Description

On 7/15/1995 the decedent was found unresponsive in his bed at a convalescent home. He was transported to a local hospital where he was pronounced dead. Per the hospital records the decedent had a history of a closed head injury "a few years prior", unknown circumstances. The decedent was receiving medical care under a false name, DOB, and social security number.
