EDD IS ACTUALLY 6-30-94 On 06/13/1994 @ 1120 hours the decedent walked into a County Welfare office @ 813 E. 4th Place in Los Angeles and doused himself with lighter fluid and set himself on fire. Transported to a local hospital where he remained until his death.
